A life insurance monitoring agency reports that North Dakota is the unhealthiest state in the nation.

PolicyGenius says they looked over life insurance applications from Nov. 2015 through Nov. 2017 to look at reported health conditions such as high cholesterol, high blood pressure, tobacco use, sleep apnea, asthma and depression.

Montana and Wyoming, which were ranked as the healthiest states in the U.S. only had 5.7 percent of applicants report these health conditions.

However, 12.4 percent of North Dakotans reported having at least one of those health conditions.

Additionally, 34.8 percent of North Dakota applicants reported tobacco use, significantly higher than the national average of 20.5 percent.

The five healthiest states were:

1. Montana
2. Wyoming
3. Utah
4. South Dakota
5. California

The five least healthy states were:

1. North Dakota
2. Alabama
3. South Carolina
4. Kansas
5. Louisiana
